As per patient, this has been present for 6-7 years, however, has grown in size and is now painful when laying on a hard surface. TECHNIQUE: Multiple routine MR pulse sequences were obtained of the chest before and after the use of IV contrast. 15 mL Magnevist was administered intravenously without adverse event. SEDATION: NONE FINDINGS: The localizer images were reviewed. Limited evaluation of the right lung, thoracic spine, spinal canal and aorta demonstrate no acute changes. Small bilateral cysts are noted within the neuroforamina, image 32 of series 8 with additional small cyst noted within the right neural foraminal canal, image 20, involving mid to lower thoracic vertebral bodies, most consistent with perineural root sleeve cysts. There is no associated contrast enhancement. Along the right upper back, there is redemonstration of a lobular, fat signal mass with multiple thin internal septations. Mass extends along the margins of the right trapezoid fascia, inferolaterally to the level of the right latissimus dorsi. This is difficult to measure given its extent along the back wall, however, when measured in coronal and axial sequences, this measures up to 15 x 6.8 x 2.0 cm in its greatest dimensions. There is mildly increased signal within the superior lateral aspect of the fatty components of the mass on both precontrast and postcontrast sequences of uncertain significance. No enhancing soft tissue element is identified. No discrete nodular or solid-appearing component is identified. There is no cystic component. There is a moderate-sized vessel coursing through the mass and extending superficially near the skin surface. IMPRESSION: MRI CHEST WITHOUT AND WITH IV CONTRAST. 1. A 15 x 6.8 x 2.0 cm probable lipoma with somewhat atypical features. No focus of soft tissue enhancement identified to suggest liposarcoma. However, given history of interval enlargement and development of pain, recommend surgical consult. NOTE: Results communication documented in primordial.
